Formstack Open DNS Support Issues
This morning, Open DNS, a “provider of Internet navigation and security”, began blocking Formstack forms from their servers. Because of Open DNS’ actions, a few of our users have reported their forms are now displaying phishing warnings.
This is not a site-wide issue. We have identified and taken down the offending account and expect to resolve things with Open DNS very soon.
Problems were first reported around 4AM Eastern time, coming to us via our Twitter account. We are working closely with Open DNS as well as with Google to ensure this does not happen again.
Get Your Forms Back Online Quickly & Easily
If you or one of your users is affected by Open DNS’ action, you can quickly bring your forms back online with the following steps.
If you are using a Formstack.com hosted form, simply replace Formstack.com with our IP address:
Change: http://www.formstack.com/forms/?xxxxxx-xxxx
To: http://64.225.152.238/forms/?xxxxx-xxxx
If you’re embedding our form in your website using our javascript embed code, you can do the same thing:
Change: <script type=”text/javascript” src=”http://www.formstack.com/forms/js.php?xxxxx-xxxx-v2″></script><noscript><a href=”http://www.formstack.com/forms/?xxxxx-xxxx” title=”Online Form”>Online Form – My Form Name</a></noscript>
To: <script type=”text/javascript” src=”http://64.225.152.238/forms/js.php?xxxxx-xxxx-v2″></script><noscript><a href=”http://64.225.152.238/forms/?xxxxx-xxxx” title=”Online Form”>Online Form – Test Form</a></noscript>
Your forms will be back in action with no phishing warnings.

I just noticed today that our forms are not working with the hardcoded IP address. I changed them back to http://www.formstack.com and they work again.
